Pfizer Health Solutions (PHS) understands the need to provide affordable, coordinated, prevention-oriented healthcare around the globe and has partnered with healthcare organizations beyond domestic borders to implement patient-centered chronic care management programs.
The more that we can share best practices, the better chance we have of improving healthcare for all of us. We are in the process of developing and testing interventions in other countries, and are incorporating national and local guidelines that build on our successful care management models. Our international projects will target patients with heart failure, congestive heart disease, and diabetes. As in all of our programs, we hope to improve clinical outcomes while increasing self-management and self-efficacy skills and promoting appropriate use of healthcare services.
