"As a physician who provides much needed quality care to the medically underserved, I personally can vouch for this program and the impact it continues to have on the lives of real people in our community. Through the efforts of Florida: A Healthy State, patients are able to learn the tools necessary to adequately manage their chronic illnesses in ways that emergency rooms and acute inpatient hospital stays can never achieve. And it is because of the personal level of the training and the learning processes that this program is able to succeed."

Florida: A Healthy State
Pfizer Health Solutions (PHS) Medicaid projects provide patients with the resources they need to manage complicated chronic medical conditions. Chronic illnesses are a key concern, particularly among the Medicaid population, because they are responsible for a large percentage of healthcare spending. Much of this can be attributed to unnecessary use of the emergency rooms and hospitalizations by patients whose chronic illnesses are unmanaged. In addition, many Medicaid patients do not have established relationships with primary care physicians and do not know where to turn for non-urgent medical attention. PHS Medicaid projects can help mitigate these factors.
Our Medicaid projects are designed to bring the best possible healthcare management programs to beneficiaries by utilizing a community-based approach to care management and providing health literacy studies and interventions. By providing the most advanced technologies and management skills, patients receive appropriate care, and the healthcare system can realize savings.
