“Humana and Pfizer Health Solutions formed Green Ribbon Health to help personalize the healthcare experience for Medicare beneficiaries as they manage chronic conditions and life transitions. Using a patient-centered approach to care management, this Medicare Health Support pilot offers a personal 24/7 nurse and a community-based, multi-disciplinary healthcare team to selected fee-for-service Medicare beneficiaries in Central Florida who have diabetes and/or congestive heart failure. This collaboration is an exciting step toward improving health outcomes and reducing cost inefficiencies in the healthcare system."

In 2000, the Centers for Medicare & Medicaid Services (CMS) selected 16 sites across the country to join a five-year Medicare Coordinated Care Demonstration project that would measure the health and financial benefits of proactive care management. PHS partnered with two of these sites to implement innovative care management programs that treat Medicare patients with chronic diseases. Our involvement in these projects will help CMS test whether these programs can improve medical treatment plans, reduce avoidable hospital admissions, and promote other desirable outcomes for beneficiaries -- without increasing program costs.

As part of the 2003 Medicare Modernization Act (Section 721), CMS authorized development and testing of voluntary Medicare Health Support pilot programs to improve the quality of care and the quality of life for people living with multiple chronic illnesses. This new initiative will help reduce health risks, improve quality of life, and provide healthcare savings.
