
Healthier Florida is a new comprehensive disease management program for chronically ill fee-for-service Medicaid beneficiaries designed to improve the health of Florida’s Medicaid population and reduce health care spending in the state.
Pfizer Health Solutions was awarded the contract to implement Healthier Florida through a competitive bidding process initiated by Florida’s Agency for Health Care Administration (AHCA).
Launched in January, 2007, Healthier Florida, builds on the success of Florida: A Healthy State, a partnership between AHCA and Pfizer that helped improve the health of more than 180,000 Medicaid beneficiaries while saving the state millions of dollars.
Healthier Florida gives participants access to a care manager who oversees their overall health issues, as well as receive assistance and support from multi-lingual community health workers, social workers, pharmacists and dieticians who work closely with community hospitals , civic organizations and patient advocacy groups.
Healthier Florida will deliver care to more than 60,000 Medicaid beneficiaries with chronic diseases, including heart failure, diabetes, hypertension, asthma, end stage renal disease (ESRD), chronic obstructive pulmonary disease (COPD) and sickle cell anemia. Healthier Florida services will be:
Program participants are connected with a “care manager” who monitors their health and links them with a local network of healthcare providers and services, as well as provides culturally relevant health education;
Participants are provided assistance and support from bilingual community health workers, social workers, pharmacists and dieticians who will work closely with community hospitals, civic organizations and patient advocacy groups; additionally, a 24-hour nurse call center will continue to operate under the new program;
Local provider relationships are leveraged to establish a team approach for coordinating care and enhancing patients’ ability to interact with their healthcare providers.
